| IQMS 2007: BASELINE EVALUATIONS OF FIRST-TIME IMPLEMENTERS - INSTITUTION- BASED PSYCHOLOGISTS AND THERAPISTS |

This minute concerns the baseline evaluations of psychologists and therapists who are newly qualified, are in their first appointments in the department, have been out of the system and returned, or are in temporary appointments.

The baseline evaluations for all institution-based therapists and psychologists who started implementing the Integrated Quality Management System (IQMS) in 2007 should now be completed.

| 3. |
The individual scores of such psychologists and therapists should be recorded on the Composite Score Sheet for Individual Educators (Form QA5i).
|
| 5. |
These documents must be submitted to the IQMS co-ordinators at the EMDC by the principals of the relevant institutions on or before Friday, 20 April 2007.
