1. The Western Cape Education Department (WCED) has revised its Exit Management Questionnaire, originally issued in 2011, so as to align it to the Department of Public Service Administration's (DPSA) Ministerial Directive to Institutionalise the Practice of Exit Interviews in the Public Service.
2. The above-mentioned ministerial directive indicates that all departments are required to compile an exit management analysis report on an annual basis for employees whose services are terminated on account of ill-health or injury, retirement and resignation.
3. In order to comply with the DPSA's directive, all employees who leave the employ of the WCED as a result of ill-health or injury, retirement and resignation should complete the attached Exit Management Questionnaire. The completed questionnaires will be analysed to determine why employees leave the public service and what possible corrective measures the WCED can implement to retain skilled and competent employees.
4. Please note that employees should complete all fields on the questionnaire to make the analysis as informative as possible. The information provided in the exit questionnaire will be treated in the strictest confidence.
5. Once the exit questionnaire has been completed, kindly forward it to the Directorate: Strategic People Management (Attention: Ms Charné Jones): 8th Floor, Golden Acre Building, Adderley Street, Cape Town, 8001.
