Kindly note that the closing date for applications for the above-mentioned concessions has been changed to 30 March 2009. For your convenience, an amended application form is attached.

The following requirements, as earlier stipulated in Circular 0013/2008, are brought to your attention once again:

A certified photocopy of the candidate's identity document must accompany all applications.
|
| 3.2 |
Supporting documentation must accompany all applications, i.e. detailed medical report, examination results, and samples of handwriting and spelling, where applicable.

| 3.3 |
Applications for additional examination writing time as a result of unforeseen circumstances, such as the fracture to an arm or finger, that occur after the above-mentioned closing date, must also be submitted on the prescribed form. In such a situation, the application form, medical certificates and other supporting documents must be faxed to the Department as soon as possible, for the attention of Mr H.A.J. Coetzee at fax number 021 461 5637 or 086 696 8927.

In the event of such unforeseen circumstances, time is an important factor. Principals and teachers are therefore asked to do the following to expedite the process:

Call Mr Coetzee at tel. no. 021 467 2921 and agree on the time and date for the fax to be sent, so as to ensure immediate attention to the application.

Thereafter, all the relevant documents must be either delivered or posted, as soon as possible, to the following address:
