According to the management plan handed out to all institution-based psychologists and therapists during training in 2005, baseline evaluations should have been completed by 15 March 2006.

This letter serves to inform all principals who have institution-based psychologists and therapists in their schools (and for whose appraisals they have taken responsibility) that the following documents must be submitted for the baseline evaluations:

The Directorate: Quality Assurance will collate the information required for administrative purposes and then provide the IQMS co-ordinators in the districts with copies of the documents for their records. Please note that this is the process only for this phase of the first cycle.

If you have already submitted the score sheets of your staff to the Directorate: Quality Assurance, we will complete the above-mentioned template on your behalf.

Kindly note that the Directorate: Quality Assurance will inform you in due course of the templates to be used for the summative evaluations due in November 2006.
