According to the management plan, all institution-based psychologists and therapists must now be completing their summative evaluation.

This letter serves to inform all principals who have institution-based psychologists and therapists in their schools that the following documents must be submitted for the summative evaluations. In the case of a psychologist or therapist who is working in a number of schools, the forms must be completed by the principal of the school in which the most time is spent, or as agreed upon by all stakeholders.

The individual scores of the psychologists and therapists should be recorded on the Composite Score Sheet (Annexure A).

The attached Summary Composite Score Sheet should be completed (Annexure B).
|
| 5. |
The documents should reach the IQMS co-ordinator at the EMDC on or before Friday 30 November 2006.

| 6. |
The details of the IQMS co-ordinators are attached (Annexure C).
