1. The procedure for the submission of leave forms is set out in Circular 8/2005 of 2 February 2005.

2. To ensure the efficient processing of applications for leave, principals and supervisors are reminded of the following:
2.1 Leave forms must be submitted in duplicate to the WCED. This will enable the WCED, once the application has been processed, to return the duplicate application form to the principal or supervisor for record purposes.
2.2 An application for leave must be submitted before the employee goes on leave. If this is not possible, such as in unforeseen circumstances, principals and supervisors must ensure that the completed leave form, with their recommendation, reaches the relevant sub directorate no later than 14 days after the employee concerned submitted the form. The submission of leave forms per month or per term does not meet these requirements.
2.3 The completion of leave forms and the submission thereof is the responsibility of the employee concerned. Employees may not ask colleagues or administrative personnel to obtain, complete and submit leave forms on their behalf. For example: A teacher at a school may not ask the school secretary to apply for leave on his or her behalf.

3. This circular is not applicable to the addendums which employees complete and submit when applying for temporary incapacity leave.

4. Kindly bring the content of this circular to the attention of all employees who report to you.
