1. Under cover of Circular: 0036/2004, the Western Cape Education Department (WCED) introduced the current exit questionnaire to be completed by educators leaving the employ of the WCED. The questionnaire does not make provision for public service employees.
2. The WCED has now revised the exit questionnaire to accommodate both educators and public servants leaving the employ of the WCED. The Department wishes to obtain as much information as possible about the reasons why educators and public servants leave the WCED, and also to obtain their views on the culture and working environment of the Department. In this way, the department can attempt to deal with certain employment-related matters and take preventative action where possible. The information received will be treated as strictly confidential.
3. As soon as line managers or principals are informed that an educator or public servant will be leaving the employ of the department, they should request that employee to complete the attached exit questionnaire (Annexure A).
4. The completed forms must be submitted to the respective director at Head Office or district office, who will then post the forms to the Director: Human Resource Management, Private Bag X9114, Cape Town, 8000, or fax them to 021 467 2019.
5. Please bring the content of this circular to the attention of all employees.
