1. You are referred to Circulars 0020/2002 of 14 February 2002 and 0120/2003 of 7 May 2003 concerning the need to deposit all salary and related payments directly into employees’ bank accounts.
2. Certain irregularities relating to the use of the Z56 form (an application form requesting one’s salary to be deposited directly into a bank account) have obligated the Western Cape Education Department to design a new application form. (Copy attached.)
3. Kindly note that, as from the date of distribution of this circular, only the new application form (Application to Deposit Salary in Bank Account) will be accepted and that all other application forms will be regarded as invalid.
4. Heads of institutions must ensure that the right-hand thumb print of any newly appointed staff member, staff member re-appointed after a break in service, or any staff member who is required to make a change in his or her banking details, appears on the application form and that the application form is accompanied by a certified copy of the staff member’s identity document. The staff members are to ensure that the original completed form, completed in their own handwriting, is submitted, as no copies or facsimiles will be accepted.
5. The contents of this circular must please be brought to the attention of all employees.
